For Kindergarten - Your child must be 5 years old by Sept 15, 2013.

For Preschool  -     Children that are 4 years old and have registered for fall

                               preschool do not need to attend.  

Bring your child’s birth certificate or other document showing age and date of birth, immunization card and the Iowa Department of Public Health dental screening form. A dental hygienist from Trinity Muscatine Public Health will be on site to perform the required dental screening.
